General coding guidelines in ICD-10-CM instruct that codes describing symptoms and signs are acceptable for reporting when the provider has not established a related, definitive (confirmed) diagnosis.
Chapter 18 of ICD-10-CM, Symptoms, Signs, and Abnormal Clinical and Laboratory Findings, Not Elsewhere Classified (codes R00.0–R99) contains many (but not all) codes for symptoms.
In determining the first-listed diagnosis the coding conventions of ICD-10-CM, as well as the general and disease specific guidelines take precedence over the outpatient guidelines. Diagnoses often are not established at the time of the initial encounter/visit.
However, the Alphabetical Index doesn’t include coding instructions, which are in the Tabular List. The Tabular List of ICD-10 codes (plus their descriptors) is organized alphanumerically from A00.0 to Z99.89. It is divided into chapters based on body part or condition.
O03.4ICD-10 Code for Incomplete spontaneous abortion without complication- O03. 4- Codify by AAPC.
ICD-10-CM Code for Complete or unspecified spontaneous abortion without complication O03. 9.
Commonly reported CPT codes for miscarriages include: 59812, treatment of incomplete abortion, any trimester. 59820, treatment of missed abortion, completed surgically; first trimester. 59821, treatment of missed abortion, completed surgically; second trimester.
Encounter for other specified aftercareICD-10 code Z51. 89 for Encounter for other specified aftercare is a medical classification as listed by WHO under the range - Factors influencing health status and contact with health services .
Inevitable miscarriage refers to the presence of an open internal os in the presence of bleeding in the first trimester of pregnancy. Most often the conception products are not expelled and intracervical contents are present at the time of examination.
ICD-10 code O36. 80X0 for Pregnancy with inconclusive fetal viability, not applicable or unspecified is a medical classification as listed by WHO under the range - Pregnancy, childbirth and the puerperium .
When an induced abortion is performed by dilating the cervix and performing sharp and/or suction curettage, use CPT code 59840 for reported such procedures. If the cervix is dilated and the uterus mechanically evacuated, code 59841 is reported.
90-dayCPT 59820 has a 90-day global period.
A missed abortion is a nonviable intrauterine pregnancy that has been retained within the uterus without spontaneous abortion. Typically, no symptoms exist besides amenorrhea, and the patient finds out that the pregnancy stopped developing earlier when a fetal heartbeat is not observed or heard at the appropriate time.
Z codes are for use in any healthcare setting. Z codes may be used as either a first-listed (principal diagnosis code in the inpatient setting) or secondary code, depending on the circumstances of the encounter.
ICD-10-PCS will be the official system of assigning codes to procedures associated with hospital utilization in the United States. ICD-10-PCS codes will support data collection, payment and electronic health records. ICD-10-PCS is a medical classification coding system for procedural codes.
I63. 9 - Cerebral infarction, unspecified | ICD-10-CM.
It is divided into chapters based on body part or condition. Most ophthalmology codes are in chapter 7 (Diseases of the Eye and Adnexa), but diabetic retinopathy codes are in chapter 4 (Endocrine, Nutritional, and Metabolic Diseases). Order the lists today.

This means that ICD-10 doesn’t include pseudopterygium as part of any condition represented by the H11.1- codes, but it is possible for a patient to have both at the same time—and if that’s the case with your patient, you would submit the relevant H11.1- code along with H11.81.
The conventions for the ICD-10-CM are the general rules for use of the classification independent of the guidelines. These conventions are incorporated within the Alphabetic Index and Tabular List of the ICD-10-CM as instructional notes.
Counseling Z codes are used when a patient or family member receives assistance in the aftermath of an illness or injury, or when support is required in coping with family or social problems.
More than one external cause code is required to fully describe the external cause of an illness or injury. The assignment of external cause codes should be sequenced in the following priority:
Code assignment is based on the provider’s documentation of the relationship between the condition and the care or procedure, unless otherwise instructed by the classification. The guideline extends to any complications of care, regardless of the chapter the code is located in. It is important to note that not all conditions that occur during or following medical care or surgery are classified as complications. There must be a cause-and-effect relationship between the care provided and the condition, and an indication in the documentation that it is a complication. Query the provider for clarification, if the complication is not clearly documented.
The word “with” or “in” should be interpreted to mean “associated with” or “due to” when it appears in a code title, the Alphabetic Index (either under a main term or subterm), or an instructional note in the Tabular List. The classification presumes a causal relationship between the two conditions linked by these terms in the Alphabetic Index or Tabular List. These conditions should be coded as related even in the absence of provider documentation explicitly linking them, unless the documentation clearly states the conditions are unrelated or when another guideline exists that specifically requires a documented linkage between two conditions (e.g., sepsis guideline for “acute organ dysfunction that is not clearly associated with the sepsis”).For conditions not specifically linked by these relational terms in the classification or when a guideline requires that a linkage between two conditions be explicitly documented, provider documentation must link the conditions in order to code them as related.
When assigning a chapter 15 code for sepsis complicating abortion, pregnancy, childbirth, and the puerperium, a code for the specific type of infection should be assigned as an additional diagnosis. If severe sepsis is present, a code from subcategory R65.2, Severe sepsis, and code(s) for associated organ dysfunction(s) should also be assigned as additional diagnoses.
code from subcategory O9A.2, Injury, poisoning and certain other consequences of external causes complicating pregnancy, childbirth, and the puerperium, should be sequenced first, followed by the appropriate injury, poisoning, toxic effect, adverse effect or underdosing code, and then the additional code(s) that specifies the condition caused by the poisoning, toxic effect, adverse effect or underdosing.
Chapter 18 of ICD-10-CM, Symptoms, Signs, and Abnormal Clinical and Laboratory Findings, Not Elsewhere Classified (codes R00.0–R99) contains many (but not all) codes for symptoms.#N#Chapter 18 also includes codes for Symptoms, Signs and Abnormal Clinical and Laboratory Findings Not Elsewhere Classifiable, for ill-defined conditions where no diagnosis classifiable elsewhere is recorded. These conditions are represented through the range of R00-R59. They consist of categories for:
A symptom code is used with a confirmed diagnosis only when the symptom is not associated with that confirmed diagnosis. It’s the coder’s responsibility to understand pathophysiology (or to query the provider), to determine if the signs/symptoms may be separately reported or if they are integral to a definitive diagnosis already reported.
Signs and symptoms associated routinely with a disease process should not be assigned as additional codes, unless otherwise instructed by the classification. Additional signs and symptoms that may not be associated routinely with a disease process should be coded, when present. Author. Recent Posts.
For G89.2- Chronic pain, not elsewhere classified, the ICD-10-CM tabular list provides separate codes for pain due to trauma, post-thoracotomy pain, other postprocedural pain, and other pain. But what you won’t find is a definition for chronic. The OGs tell you that “There is no time frame defining when pain becomes chronic pain. The provider’s documentation should be used to guide use of these codes” (Section I.C.6.b.4).
The general rule is that you should sequence the G89.- pain code first when the reason for the admission or encounter is pain control or pain management. Here are some examples:
ICD-10-CM is picky about pain coding, and it’s no wonder considering all the possible variations. Fortunately you can turn to the 2019 ICD-10-CM Official Guidelines for Coding and Reporting (OGs) to keep your coding skills sharp. Today we’ll take a look at some of these pain pointers.
For abortion, another term “interrupted pregnancy” is often used. It refers to a pregnancy that did not proceed to full term. A full-term pregnancy is normally between 38 and 42 weeks of gestation. There are numerous ways to classify an interrupted pregnancy. It can be an abortion or it can be a miscarriage.
There are numerous ways to classify an interrupted pregnancy. It can be an abortion or it can be a miscarriage. Abortion or miscarriage both are considered as natural death of an embryo or fetus. A miscarriage is one of the most common complications of early pregnancy.
