Is pain under my left arm after surgery a concern?

Yes, issues with shoulders are common. I had issues with my left prior to surgery but since surgery its really bad. The fact the movement to the side is when it hurts is indicative of a rotator cuff issue or as Tom says, impingement. Try the exercises and hopefully it will get better; however, I am 18 mos post surgery and mine has not improved.
