The case cited describes a significant procedure performed on the vein in order to retrieve the IVC filter (‘the right internal jugular vein was punctured and dilated’). The following combination of codes should be used to classify this procedure:

Chapter 21 of ICD-10-CM (Factors Influencing Health Status and Contact with Health Services) contains codes for insertion and routine removal of CVCs. For a hemodialysis catheter, the appropriate code is Z49. 01 (Encounter for fitting and adjustment of extracorporeal dialysis catheter). For any other CVC, code Z45.
The procedure to remove the IVC filter is similar to the procedure used to place it. A small catheter-based wire loop (snare) is inserted into the large vein in the neck. A removable IVC filter contains a small hook at one end. With X-ray guidance, your doctor uses the snare to grasp the hook and withdraw the filter.Feb 22, 2022

Angioplasty is a common procedure performed to improve blood flow in arteries or veins that have become narrowed or blocked. In the interventional radiology area, these procedures are performed by percutaneous technique using specially designed balloon catheters.
Vascular filters are placed in the inferior vena cava (IVC) in patients at risk of pulmonary embolism from known deep vein thrombosis. In the interventional radiology area, these filters are placed via percutaneous transcatheter approach. Special catheters containing a pre-loaded filter are inserted into the IVC via femoral vein or internal jugular approach and the filter is deployed. The filter grips the walls of the vena cava and is designed to “catch” clots migrating from the lower extremities. Filters may be temporary or permanent.
