Laparoscopic liver biopsy provides direct visualization that increases diagnostic accuracy when a tissue sample is needed from a specific area of the liver. The laparoscope is inserted through a small abdominal incision to project the image of the liver onto a monitor. Instrumentation is passed through the laparoscope or a separate small incision. By using the images, the physician is able to retrieve samples of liver tissue. Laparoscopic biopsy, like transjugular liver biopsy, may be performed in the presence of ascites or coagulation disorders.
Liver biopsy is the most reliable way of obtaining histiological information on the condition of the liver. Abnormal liver biopsy results may confirm alcoholic liver disease, cancer of the liver, hepatitis, hemochromatosis and cirrhosis.
When access is obtained via the femoral vein instead of the jugular vein, the biopsy may be referred to as transvenous liver biopsy. The procedure takes about 30-60 minutes and requires electrocardiographic monitoring to detect any arrhythmia that may be induced by introduction of the catheter. Transjugular liver biopsy may be performed when there is contraindication to conventional percutaneous liver biopsy due to coagulopathy, ascites and morbid obesity. Although the tissue sample may be smaller and more fragmented than the tissue acquired with the percutaneous method, the transjugular procedure is safe and provides useful diagnostic information.
When a transjugular liver biopsy is performed, a catheter is advanced into the right internal jugular vein, through the right atrium and into the right hepatic vein. Once contrast material confirms position, tissue samples of the liver parenchyma are obtained through a needle that is passed through... To read the full article, sign in and subscribe ...
