A bruise, also known as a contusion, typically appears on the skin after trauma such as a blow to the body. It occurs when the small veins and capillaries under the skin break. A hematoma is a collection (or pooling) of blood outside the blood vessel.
Abstract. Soft-tissue hematomas are a common clinical entity often associated with trauma, surgery, and bleeding disorders. In the majority of cases, soft-tissue hematomas acutely appear and spontaneously resolve, but sometimes, they present as swellings that slowly expand and progressively increase with time.

Hematomas, seromas and fluid collection. If you incise and drain a hematoma, seroma or fluid collection, use CPT 10140. In this procedure, you incise the pocket of fluid and bluntly penetrate it to allow the fluid to evacuate. You can use this code with or without the necessity of packing.
Hematomas cause swelling and inflammation. Often the inflammation and swelling cause irritation of adjacent organs and tissues, and cause the symptoms and complications of a hematoma. One common complication of all hematomas is the risk of infection.
Retroperitoneal hematoma is defined as bleeding into the retroperitoneal space. This clinical entity is often occult and under-recognized by clinicians and is a cause of significant morbidity and mortality.
The area in the back of the abdomen behind the peritoneum (the tissue that lines the abdominal wall and covers most of the organs in the abdomen). The organs in the retroperitoneum include the adrenal glands, aorta, kidneys, esophagus, ureters, pancreas, rectum, and parts of the stomach and colon.
An abdominal hematoma can be intrabdominal or an abdominal wall hematoma. Abdominal wall hematoma usually results from bleeding inside the muscle layers of the abdominal wall, most commonly the vascular rectus muscle. A known category of this hematoma is rectus sheath hematoma.

Since you specifically described opening and draining the fluid space, the correct code would be CPT 10140 (incision and drainage of hematoma, seroma or fluid collection). The performance of a nail debridement can, on some occasions, spontaneously incise/open an existing subungual fluid space.

The overall rate of perioperative hemorrhage or hematoma is 2.25 per 1,000 admissions. If a hospital had a rate of 0, one might suspect tomfoolery.
I am not sure why it is titled “perioperative hemorrhage or hematoma,” because the included diagnoses are all “postprocedural” hemorrhages or hematomas. There is a whole set of intraoperative hemorrhage and hematoma ICD-10-CM codes, but they are not included in PSI-9. In fact, I think this introduces one of the elements of confusion. It is possible that some are conflating PSI-9 with PSI-15, abdominopelvic accidental puncture or laceration rate. The conditions that fall under PSI-15’s purview are intra operative misadventures of puncture or laceration.
If there are dense adhesions and an enterotomy is inadvertently caused, was it unavoidable? If there is hemorrhage because the neoplasm is excessively vascular, the bleeding is inherent to the pathology, and not directly caused by the surgery. If a patient undergoes a thoracotomy, an iatrogenic pneumothorax is expected, and is excluded from PSI-6.
