ICD-9-CM codes are no longer used as of Oct. 1, 2015, so look to the ICD-10-CM codebook when choosing a diagnosis code. There is no code to indicate the patient is “at risk” for deep vein thrombosis (DVT) prophylaxis.
Z29.9ICD-10 code Z29. 9 for Encounter for prophylactic measures, unspecified is a medical classification as listed by WHO under the range - Factors influencing health status and contact with health services .

ICD-10-CM Code for Long term (current) use of anticoagulants Z79. 01.
DVT prophylaxis can be primary or secondary. Primary prophylaxis is the preferred method with the use of medications and mechanical methods to prevent DVT. Secondary prophylaxis is a less commonly used method that includes early detection with screening methods and the treatment of subclinical DVT.
The manufacturer recommends dosing enoxaparin for VTE prophylaxis 40 mg subcutaneously (SC) once daily for the general population or 30 mg SC twice daily in orthopedic surgery patients.
Lovenox (enoxaparin sodium) Injection is an anticoagulant (blood thinner) used to prevent blood clots that are sometimes called deep vein thrombosis (DVT), which can lead to blood clots in the lungs. A DVT can occur after certain types of surgery, or in people who are bed-ridden due to a prolonged illness.
Current guidelines suggest that anticoagulants be continued indefinitely in unprovoked VTE patients with nonhigh bleeding risk. If a patient has a yearly bleeding risk on anticoagulants > 3% (ie, high bleeding risk), we would expect a 20-year cumulative risk for major bleed of ∼60%.
Long term (current) use of anticoagulants Z79. 01 is a billable/specific ICD-10-CM code that can be used to indicate a diagnosis for reimbursement purposes. The 2022 edition of ICD-10-CM Z79. 01 became effective on October 1, 2021.
ICD-10-CM Diagnosis Code D68 D68.
Acute embolism and thrombosis of unspecified deep veins of unspecified lower extremity. I82. 409 is a billable/specific ICD-10-CM code that can be used to indicate a diagnosis for reimbursement purposes. The 2022 edition of ICD-10-CM I82.
What Is Lovenox? Lovenox (enoxaparin sodium) Injection is an anticoagulant (blood thinner) used to prevent blood clots that are sometimes called deep vein thrombosis (DVT), which can lead to blood clots in the lungs.
For deep vein thrombosis (DVT), relative to low LMWH, DOACs were the most effective at preventing VTE, resulting in 53 to 139 fewer VTE events per 1,000 patients.
The 2022 edition of ICD-10-CM Z79.01 became effective on October 1, 2021.
Z77-Z99 Persons with potential health hazards related to family and personal history and certain conditions influencing health status
Z29.9 is a billable diagnosis code used to specify a medical diagnosis of encounter for prophylactic measures, unspecified. The code Z29.9 is valid during the fiscal year 2021 from October 01, 2020 through September 30, 2021 for the submission of HIPAA-covered transactions. The code is exempt from present on admission (POA) reporting for inpatient admissions to general acute care hospitals.#N#The code Z29.9 describes a circumstance which influences the patient's health status but not a current illness or injury. The code is unacceptable as a principal diagnosis.#N#Unspecified diagnosis codes like Z29.9 are acceptable when clinical information is unknown or not available about a particular condition. Although a more specific code is preferable, unspecified codes should be used when such codes most accurately reflect what is known about a patient's condition. Specific diagnosis codes should not be used if not supported by the patient's medical record.
Unspecified diagnosis codes like Z29.9 are acceptable when clinical information is unknown or not available about a particular condition. Although a more specific code is preferable, unspecified codes should be used when such codes most accurately reflect what is known about a patient's condition.
Z29.9 is exempt from POA reporting - The Present on Admission (POA) indicator is used for diagnosis code s included in claims involving inpatient admissions to general acute care hospitals. POA indicators must be reported to CMS on each claim to facilitate the grouping of diagnoses codes into the proper Diagnostic Related Groups (DRG). CMS publishes a listing of specific diagnosis codes that are exempt from the POA reporting requirement. Review other POA exempt codes here.

Z29 is a non-billable ICD-10 code for Encounter for other prophylactic measures. It should not be used for HIPAA-covered transactions as a more specific code is available to choose from below.
A “code also” note instructs that two codes may be required to fully describe a condition, but this note does not provide sequencing direction. The sequencing depends on the circumstances of the encounter.
Position patients in a supine position for Lovenox administration by deep subcutaneous injection.
The minimum amount of benzyl alcohol at which serious adverse reactions may occur is not known. Lovenox multiple-dose vials contain 15 mg/mL of benzyl alcohol (at the dose of 1.5 mg/kg twice a day, benzyl alcohol exposure in patients is 0.45 mg/kg daily) [see Warnings and Precautions (5.8)].
When administered in conjunction with a thrombolytic (fibrin specific or non–fibrin specific), administer Lovenox between 15 minutes before and 30 minutes after the start of fibrinolytic therapy. The usual duration of Lovenox therapy is 8 days or until hospital discharge.
Placement or removal of a catheter should be delayed for at least 12 hours after administration of lower doses (30 mg once or twice daily or 40 mg once daily) of Lovenox and at least 24 hours after the administration of higher doses (0.75 mg/kg twice daily, 1 mg/kg twice daily, or 1.5 mg/kg once daily) of Lovenox.
Lovenox is indicated for: the inpatient treatment of acute deep vein thrombosis with or without pulmonary embolism, when administered in conjunction with warfarin sodium. the outpatient treatment of acute deep vein thrombosis without pulmonary embolism when administered in conjunction with warfarin sodium.
These agents include medications such as: anticoagulants, platelet inhibitors including acetylsalicylic acid, salicylates, NSAIDs (including ketorolac tromethamine), dipyridamole, or sulfinpyrazone. If coadministration is essential, conduct close clinical and laboratory monitoring [see Warnings and Precautions (5.1)].
Do not administer Lovenox by intramuscular injection.
That would be a hx of DVT with the code for long term or current drug, such as Z79.51 for anti coagulant treatment.
There is no code to indicate the patient is “at risk” for deep vein thrombosis (DVT) prophylaxis. The only ICD-10 codes you could use would be a positive diagnosis supported by the provider’s documentation that states the patient has the condition.
There is no code to indicate the patient is at risk for a certain dx. There are Vcodes for encounters for prophylactic treatments. The only dx codes you could use would be the symptoms documented by the provider. P.
