90460: Immunization administration through 18 years of age via any route of administration, with counseling by physician or other qualified health care professional; first or only component of each vaccine or toxoid administered.
Administer all diphtheria, tetanus, and pertussis vaccines (DT, DTaP, Td, and Tdap) by the intramuscular route. The preferred injection site in infants and young children is the vastus lateralis muscle of the thigh. The preferred injection site in older children and adults is the deltoid muscle in the upper arm.
A modifier -25 may be required for the office visit when a vaccine is administered. Modifier -25 indicates that the E/M code for the office visit represents a distinct and significant service that is separate from the vaccine administration.
In the absence of counseling, the administrations must be reported with codes 90471-90474. 90460-90461 are reported for administration to patients 18 years of age and under. Code 90460 is reported for each separate administration of single component vaccines and/or first component of a combination vaccine.
Of these, only two of the immunization administration codes, 90460 and 90461, include counseling by the physician or other qualified health care professional (QHP) at the time of service, specifically for patients up to and through 18 years of age.
