A patient underwent BAL of the left upper lobe bronchus via bronchoscopy. Assign ICD-10-PCS code (s): 3E1F88Z 0B9G8ZX 0BB88ZX 0B988ZX. Answer: OB9G8ZK. According to Coding Clinic 1Q201y page 51, a BAL is a bronchoalveolar lavage and does not include the entire lung or both lungs.
Most clients do not code the EBUS in ICD-10-PCS. For CPT this procedure is coded: 31628, Bronchoscopy, rigid or flexible, including fluoroscopic guidance, when performed, diagnostic, with cell washing, when performed; with transbronchial lung biopsy(s), single lobe. 14
3. A patient underwent BAL of the left upper lobe bronchus via bronchoscopy. Assign ICD-10-PCS code (s): 3E1F88Z 0B9G8ZX 0BB88ZX 0B988ZX Answer: OB9G8ZK. According to Coding Clinic 1Q201y page 51, a BAL is a bronchoalveolar lavage and does not include the entire lung or both lungs. BAL is a liquid biopsy so is coded to root operation "drainage" and
For ICD-10-PCS this procedure is coded: 0BB48ZX for excision of right upper lobe bronchus via bronchoscopy for diagnostic purposes. (This includes both the endobronchial biopsies and brushings of the RUL) "Brushings" are coded to the root operation "Excision" and "lavage" is coded to the root operation "Drainage."
Bronchoscopy with transbronchial biopsy is a procedure in which a bronchoscope is inserted through the nose or mouth to collect several pieces of lung tissue. Alternative Names. Biopsy - lung - bronchoscopic.
CPT code 31652 with endobronchial ultrasound (EBUS) guided transtracheal and/or transbronchial sampling (eg, aspiration[s]/ biopsy[ies]), one or two mediastinal and/or hilar lymph node stations or structures is utilized when one samples two or less proximal structures.
31628, Bronchoscopy, rigid or flexible, including fluoroscopic guidance, when performed, diagnostic, with cell washing, when performed; with transbronchial lung biopsy(s), single lobe.
An endobronchial ultrasound (EBUS) uses soundwaves to get a picture of your lungs and nearby lymph nodes from inside the windpipe. You usually have a biopsy to find out for certain if you have lung cancer.
Lung Biopsy The code 32405, “Biopsy, lung or mediastinum, percutaneous needle,” has been replaced by new code 32408, “Core needle biopsy, lung or mediastinum, percutaneous, including imaging guidance, when performed.” Accordingly, imaging guidance may no longer be billed separately.
You'll notice as well that 31622 is noted as a "separate procedure" which means it can usually only be billed when billed alone, otherwise it becomes part of the "bigger procedure" , in your case the 31625.
Answer: 31628, 31652. NCCI edits are precluding the reporting of 31625 with 31628, so only code 31628 can be reported with 31652.
CPT® Code 31622 in section: Bronchoscopy, rigid or flexible, including fluoroscopic guidance, when performed.
31625 – Bronchoscopy, rigid or flexible, with or without fluoroscopic guidance; with bronchial or endobronchial biopsy(s); single or multiple sites.
During EBUS. Your doctor inserts the bronchoscope, a thin, lighted tube, through your mouth, down your windpipe, and into the bronchi. A small camera attached to the bronchoscope lets your doctor see your airways, blood vessels, lungs, and lymph nodes on an ultrasound monitor.

Endobronchial means inside the airway. Endobronchial valves (EBV) are a treatment designed to improve breathing for patients who have emphysema. The EBV is a one way valve that controls airflow. It allows air out of the over inflated part of the lung, but does not allow air back in to it.
The oropharynx is examined, the vocal cords are visualized, the bronchoscope is then advanced to the trachea, which is also examined before the scope is advanced into each main stem bronchus, because we have a right and left bronchus, and any abnormalities are noted.
You can see the bronchoscope; it’s inserted through the mouth or the nose. They examine the pathway all the way down and then they do these washings at the end. That’s what that procedure is and how it works. So to code this particular situation, it doesn’t matter that nothing was found.
If a rigid bronchoscope is used, a telescope or a flexible bronchoscope may be inserted through the rigid bronchoscope to visualize the distal segment . Think of like this hard tube, but they can put the flexible one inside of the hard tube to go down into the real deeper recesses there.
