Prolactin levels are normally high for pregnant women and new mothers. Levels are normally low for nonpregnant women and for men. If prolactin levels are higher than normal, it often means there is a type of tumor of the pituitary gland, known as a prolactinoma. This tumor makes the gland produce too much prolactin.Dec 17, 2020
Sometimes, however, prolactin levels are high at other times; this condition is called hyperprolactinemia. Hyperprolactinemia predominately affects women, but it can cause infertility, decreased sex drive, and bone loss in both sexes.
Symptoms include irregular or absent menstrual periods, infertility, menopausal symptoms (hot flashes and vaginal dryness), and, after several years, osteoporosis (thinning and weakening of the bones). High prolactin levels can also cause milk discharge from the breasts.May 10, 2021

When Are Prolactin Levels Checked? You can have your prolactin levels checked at any point in your menstrual cycle. Prolactin levels vary throughout the day but are highest while you are asleep and first thing in the morning, so the test is usually done about three hours after you wake up.

High levels of prolactin can also result in weight gain and neuropsychological disturbances. The size of the tumor correlates with the amount of prolactin secreted. Larger tumors can cause mass effects by compression of local structures.

A prolactin level above normal obtained without excessive venipuncture-induced stress confirms the diagnosis. Prolactin levels associated with stress or dopamine antagonists are usually below 100 μg/dl. A prolactin level greater than 250 μg/dl usually indicates the presence of a prolactinoma.
Normal levels are less than 500 mIU/L [20 ng/mL or µg/L] for women, and less than 450 mIU/L for men.
