For the ICD-10 code, report Z45.010 (Encounter for checking and testing of cardiac pacemaker pulse generator [battery]), which applies to replacing the battery according to a note with the code.
If it is just repositioning the pacemaker, I would use this code. 33215 would not exclude the electrode, this code is specifically for repositioning of a previously placed right atrial or right ventricular electrode of either a single or dual chamber pacemaker or cardioverter-defibrillator system.
If the system is an ICD rather than a pacer it would be 33240 for a new ICD generator (single or dual is the same code) and 33241 for removal of the old generator, plus 33225 for the LV lead.
The guidelines say that with a battery replacement, you must code both the removal and the insertion of a pulse generator. For this question, we found code 33224, which says it includes both removal and repair. Can anyone shed some light on this for me? I am in school studying coding, and we are working on a question in class.

33227 Removal of permanent pacemaker with replacement of pacemaker; single lead system for removal of the pulse generator and a code for the insertion of the pulse generator. CPT copyright American Medical Association.
The ICD-10-PCS code assignment for this case example is:0JH606Z, Insertion of pacemaker generator.02H63JZ, Insertion of device in atrium.02HK3JZ, Insertion of device in ventricle.
33206 Insertion of new or replacement of permanent pacemaker with transvenous electrode(s); atrial.
A9. IC: Use CPT® code 33224: Insertion of pacing electrode, cardiac venous system, for left ventricular pacing, with attachment to previously placed pacemaker or implantable defibrillator pulse generator (includes revision of pocket, removal, insertion, and/or replacement of existing generator).
when coding a change of battery in a pacemaker, both the removal and the re-implantation are coded. to correctly report coronary bypass grafts, you must know the anatomical site from which the vessel being grafted came. PTCA is an acronym for percutaneous transluminal coronary angioplasty.

Dual chamber pacemaker. This type carries electrical impulses to the right ventricle and the right atrium of your heart to help control the timing of contractions between the two chambers.
33228. Removal of permanent pacemaker pulse generator with replacement of pacemaker pulse generator; dual lead system. For OPPS billing, add the HCPCS code for the implanted device: C2619.
33249. INSERTION OR REPLACEMENT OF PERMANENT IMPLANTABLE DEFIBRILLATOR SYSTEM, WITH TRANSVENOUS LEAD(S), SINGLE OR DUAL CHAMBER. 33262. REMOVAL OF IMPLANTABLE DEFIBRILLATOR PULSE GENERATOR WITH REPLACEMENT OF IMPLANTABLE DEFIBRILLATOR PULSE GENERATOR; SINGLE LEAD SYSTEM.
While there are 31 root operations in the medical and surgical section of ICD-10-PCS, there are specific root operations common to cardiovascular procedures:
The following are case examples illustrating how to code cardiovascular cases using ICD-10-PCS.
These examples provide a glimpse of several of the more common cardiovascular procedures in ICD-10-PCS. For additional exposure, consider reviewing table 021–02Y in the Heart and Great Vessels body system. Often, a review of the tables in ICD-10-PCS provides additional insight and information in procedure code assignment.
