2018/2019 ICD-10-CM Diagnosis Code H65.2. Chronic serous otitis media. H65.2 should not be used for reimbursement purposes as there are multiple codes below it that contain a greater level of detail.
Since the primary dx the dr gives is ear pain, I would assume he's talking about OM with the serous effusion dx. Since I don't have the documentation in front of me I cannot be sure but I believe you should look at 381.00/381.01 for the serous effusion (serum-fluid).
Other chronic nonsuppurative otitis media 1 Chronic exudative otitis media. 2 Chronic nonsuppurative otitis media NOS. 3 Chronic otitis media with effusion (nonpurulent). 4 Chronic seromucinous otitis media.
This is the American ICD-10-CM version of H65.2 - other international versions of ICD-10 H65.2 may differ. code for any associated perforated tympanic membrane ( H72.-)

Chronic Serous Otitis Media This condition is commonly caused by long standing Eustachian tube blockage, or from a thickening of the fluids so that it cannot be absorbed or drained down the tube. Chronic otitis media may be irritating or painless, but ear pressure and popping of the ears is often constant.

Otitis media is a generic term that refers to an inflammation of the middle ear. The middle ear is the space behind the eardrum. Otitis media with effusion means there is fluid (effusion) in the middle ear, without an infection.
Acute suppurative otitis media is distinguished from secretory (serous) otitis media by the presence of purulent fluid in the middle ear. Pathogenic bacteria may be cultured from the majority of needle aspirates of this purulent fluid.
Otitis media with effusion (OME) is a collection of non-infected fluid in the middle ear space. It is also called serous or secretory otitis media (SOM). This fluid may accumulate in the middle ear as a result of a cold, sore throat or upper respiratory infection.
