Coding for Glaucoma. Glaucoma (ICD-9-CM category 365) is a group of conditions resulting in optic nerve damage caused by increased intraocular pressure. It can cause a gradual progression of vision loss if left untreated.
5 Glaucoma secondary to other eye disorders.

V80. 1 - Screening for glaucoma. ICD-10-CM.
ICD-10 code H40. 113 for Primary open-angle glaucoma, bilateral is a medical classification as listed by WHO under the range - Diseases of the eye and adnexa .
Moderate stage glaucoma is noted with a “2” as the final digit in the diagnosis code. As in the example above, moderate stage open angle glaucoma with borderline findings and low risk the ICD-10 code would be H40. 011X2 for the right eye and H40. 012X2 for the left eye.
ICD-10 code H40. 112 for Primary open-angle glaucoma, left eye is a medical classification as listed by WHO under the range - Diseases of the eye and adnexa .
ICD-10 Code for Primary open-angle glaucoma, bilateral, moderate stage- H40. 1132- Codify by AAPC.
An eye that has lost all vision and has uncontrolled pressure is sometimes referred to as having “absolute glaucoma”. In such situations, the mechanism of underlying pressure elevation may be apparent (e.g., neovascular or uveitic glaucoma), whereas in other cases, it may not.
ICD-10 Code for Encounter for examination of eyes and vision without abnormal findings- Z01. 00- Codify by AAPC.

The GEMs are the raw material from which providers, health information vendors and payers can derive specific applied mappings to meet their needs.
Glaucoma is a group of diseases that can damage the eye's optic nerve. It is a leading cause of blindness in the United States. It usually happens when the fluid pressure inside the eyes slowly rises, damaging the optic nerve. Often there are no symptoms at first. Without treatment, people with glaucoma will slowly lose their peripheral, or side vision. They seem to be looking through a tunnel. Over time, straight-ahead vision may decrease until no vision remains.

Teaching guide. The AGS is preparing an online Teaching Set with real-world examples of glaucoma cases. This teaching guide is designed to help you test your coding skills with the new codes. Check the AGS website ( www.americanglaucomasociety.net) for a link.
The new glaucoma staging system was developed by an American Glaucoma Society (AGS) work group, which included Drs. Fellman and Mattox.
Given the great variability in costs of care and resource utilization among glaucoma patients, glaucoma care will be targeted for the use of potential value-based modifiers in coming years. To prepare for this possibility, ICD-9/ICD-10 codes reflecting disease severity will allow for stratification of a patient population in a practice.
While many CPT codes are bundled with the 65820 goniotomy code (see “CCI Bundling”), it is worth making a mental note of the 7 codes below, all of which can be unbundled when appropriate.
What does gonioscopy-assisted transluminal trabeculot omy (GATT) using a suture or iTrack microcatheter (Ellex) have in common with procedures that use the Kahook Dual Blade (New World Medical), Trab360 (Sight Sciences), or Trabectome (NeoMedix)? Per the Academy Health Policy Committee, these ab interno trabeculotomy (also known as goniotomy) techniques can be billed using CPT code 65820.
Keep in mind the following: Goniotomy should not be coded in addition to other angle surgeries or canal implants. Goniotomy treats congenital glaucoma and adult open-angle glaucomas. If using an ophthalmic endoscope, you can bill 66990 as well as 65820. Payment is per eye.
