ICD-10-PCS Draft Coding Guideline B5. 2 states that procedures performed via natural or artificial opening with percutaneous endoscopic assistance are coded to approach value F. The code for a laparoscopic-assisted total vaginal hysterectomy is 0UT9FZZ, with the fifth character value of F.
Two codes differentiate an open appendectomy without rupture (44950) and with rupture (44960). However, only one code applies to laparoscopic appendectomy (44970), and it is used to report a laparoscopic appendectomy for either scenario; with rupture or without rupture (see Table 2, page 43).
CPT code 44970 is used for laparoscopy surgical appendectomy. CPT code 44950 is used for removal of appendix (appendectomy) by abdominal incision.

47.01 Laparoscopic appendectomy - ICD-9-CM Vol. 3 Procedure Codes.
Under CPT guidelines this code would only be reported 1) when this is the only procedure performed and the appendix is removed for a diagnosis other than rupture with abscess, or 2) with a modifier -52 added if the surgeon believes that an incidental appendectomy needs to be reported.
CPT codes 44950 and 44960 are used for open primary appendectomies. Code – 44960 is used only for an appendix that has perforated or ruptured, and/or for diffuse peritonitis.

Code 44955 is the code to report when an appendectomy is performed for an indicated purpose at the time of other open abdominal procedures. For instance, the appendix may have been removed due to a finding of distention with fecalith or extensive adhesions binding the appendix to the abdominal wall.
Modifier 22 is defined as "Increased Procedural Services: When the work required to provide a service is substantially greater than typically required, it may be identified by adding modifier 22 to the usual procedure code.
Modifier -52 identifies that the service or procedure has been partially reduced or eliminated at the physician's discretion. The basic service described by the procedure code has been performed, but not all aspects of the service have been performed.
