What are the parts of a pacemaker/ICD?
Pacemaker & ICD are two different devices. While pacemaker’s job is to treat slow heart rate, ICD’s job is to treat very fast chaotic heart rhythm. Pacemaker is used when patient’s heart rate drops to dangerously low levels.
An implantable cardioverter defibrillator (ICD) looks similar to a pacemaker, though slightly larger. It works very much like a pacemaker. But the ICD can send an energy shock that resets an abnormal heartbeat back to normal. Many devices combine a pacemaker and ICD in one unit for people who need both functions.

In MOST, therefore, the diagnosis of pacemaker syndrome strictly required documentation of new symptoms (e.g., dyspnea, syncope) together with VA conduction or systolic BP reduction of 20 mm Hg or more during VVI pacing compared to intrinsic rhythm.

Presentation of pacemaker syndrome Symptoms include pulsation and fullness in the neck, dizziness, palpitations, fatigue, light-headedness and syncope. Symptoms and signs of heart failure may occur. Signs include hypotension, tachycardia, tachypnoea, raised JVP and cannon waves.
These complications include infection (4%), pneumothorax (1%), cardiac perforation and tamponade, bleeding, and pain. Atrial fibrillation. In patients with sick sinus syndrome, VVI pacing leads to a higher rate of atrial fibrillation than does physiologic (ie, AAI or DDD) pacing.
